Palliative Care Australia is the peak national organisation for palliative care. It publishes the Standards for providing quality palliative care for all Australians and a range of resources including a dedicated section, Education and training resources for residential aged palliative care. They have a very good consumer page which allows you to download information brochures and resources including a pain management diary and medicines recording chart.
Their website offers free, quality, evidence-based information on palliative care. The website has specific sections for patients, carers, families and health professionals.
An Advance Care Directive (‘Living Will’) is a written statement regarding someone’s wishes for their future health care. An Advance Care Directive can be made by anyone who has the capacity to do so. An Advance Care Directive is only used if, at some point in the future, the person becomes incapable of making health care decisions for themselves (due to illness or injury).
